In ACT! parlance, we call that an "activity". In numerous places you can schedule one - on the contact in question, select the Activities tab at the bottom, right click in the white space of the tab and choose "Schedule"...
If you want ACT! to remind you when the time comes, be sure to set the lead time in the "Ring Alarm" field at the bottom of the dialog.
Not sure why, but by default, all ACT! activities are set to not Ring alarms when you first install. If you hate this - I do - go to Preferences on the Tools menu and in the Calendar and Scheduling tab, open Scheduling Preferences and you can set your activities to default to Ring Alarm with varying lead times. Sweet!
