Have you set up the dialer in ACT! under Tools > Preferences > Communication > Dialer Preferences?
This is where you tell ACT! what modem to use.
When you've done that the Phone Contact icon should appear.

That wasn't much info to go on. Did you by any chance recently install some fax software add-on ?
