I don't know if this is a known issue but I know that there were similar issues back in 2008.
When you click the 'add note' icon to add a note record to a record, the note will link to a random record and not to the recvord that is actually being used, is there either someway to fix this or is the user doing something wrong to cause this to happen in the first place.
They are on ACT Premium 2012 SP1.
Any suggestions please wold be greatfully received.

This typically happens when they add a new contact record and add the note or history before moving to a different contact. The new contact record doesn't become the looged contact until saved and the logged contact is the previous contact.

Yes that's what I thought to but it seems to happen when adding a note to an existing record as well. THey are going through adding notes to existing records and when they go to another record and click 'new note' then the new note is related to another 'random' record.
I have suggested that they click the 'save' button after creating each note thereby locking in the change but I'm not sure that will necessarily solve the problem.

Save is good but Refresh might be better.
