It is possible that the copy of the 5160 label template has become corrupted.
You can test this by opening the Demo database and attempting to preview the 5160 template.
If it works within the Demo database, you can replace (copy/paste) your database's copy of the template with a copy of the 5160 template from the Demo database.
Here is the default location for the template on an XP machine:
C:\Documents and Settings\All Users\Documents\ACT\ACT for Windows 12\Databases\ACT2010Demo-database files\Reports\Avery 5160 - 5260 - 5560.lbl
