Act! Pro Version 18.104.22.168
Windows 10 64 bit
Office and Outlook 2016 64 bit
Running Act on desktop.
Every time I exit and restart Act, all of my menu and shortcut customizations reset and I have to start all over again.
Does anyone have a solution for this?

Do you have the latest ACT!Update? Manually download and install it from ACT! Download Center
Probably this will resolve.
DB Rebuild options.
I do this every 1 or 2 weeks.
Backup your Custom Commands Toolbar if needed.
File / Backup / Personal Files, select only the Menus Toolbars option.

ran the actdiag process
ran the missing preferences tool http://kb.act.com/app/answers/detail/a_id/21108
These did NOT resolve me issue.
someone please help as this is a deal breaker fro me

Sorry that did not work.
Will take another look see. Be sure to have recent db backup - ACT! - File - Backup database
Ok - what about this?
Something else to check;
Manually check for Windows Updates and apply if needed.

In addition to Bill's post above, I would recommend following the section in KB 14770 titled "Manually Rebuilding the Preferences".

Hold on a second.............
WHY is this happening to us customers?
WHY are we forced to have to address this?
WHAT line of ACT code is running that screws up our preferences?
WHY hasn't ACT fixed the problematic code?
GEEEEEEEEZZZZZ, I spend more time trying to get ACT to run, as it is represented pre-sale, than actually using it.
I cannot help but to wonder how many small businesses have gone bankrupt trying to fix ACT rather than using it to earn money.
We can't even rely on preferences to be stable?!!?

Under Customize Menus and Toolbars in the Toolbars tab, are check boxes for:
My "preference" is to have all of these toolbars checked so that I can use them; however, I have checked them countless times and my selections never stick after closing ACT and restarting it.
I tried your solution using Answer ID 14618 and it did not work.

to understand what's going on here, It's important to understand the differences between the two different types of toolbars in Act!:
The first kind of toolbar can be shown in any view (i.e. Contacts, Companies, Task List, Opportunities, etc.), whereas the latter can only be shown when you are in that particular view.
In the screenshot below, the example toolbars highlighted in Yellow are universal toolbars. The blue highlighted ones are view-specific.
You can see from this screenshot that I have 3 universal toolbars, with only 2 enabled. If i were to enable the third, it would appear in all views throughout the software along with the other two above it. All custom toolbars are universal.
You can also tell from the selected view-specific toolbar which view I was in when opening the Customize Menus and Toolbars window. In this case it was the Contact Detail view, and therefore it is the only one selected out of these toolbar types.
If i were to attempt to show another view-specific toolbar for a view I was not currently in (i.e. the Task List Toolbar while in Contact Detail view), nothing would happen, as this toolbar can only be shown when the Task List view is being displayed. I would also find that the next time I opened this window in the same view, the incompatible toolbar has been unselected.
In other words, it's not possible to select more than one view-specific toolbar, however the correct toolbar for the view you are currently in will be automatically selected for you when opening that view.
I have made the suggestion that the toolbar options in this window be made less confusing, and only show the view-specific toolbars that are available in the view you are currently in.
I believe this explains why you are seeing some toolbars becoming unselected when viewing the Customize Menus and Toolbars window after restarting Act!, or after re-opening the window from another view in the software. Please let us know if you think this is not the case here.
