How do I add a subject line to mail merge email templates?

Welcome to the Sage ACT! Community. You would do this on Screen 5 of the mail merge wizard (same screen as history type and attachment options). This can only be done during the merge - it cannot be added to the actual template.

I guess I've missed that screen. When I click mail merge I choose the template and then it automatically sends the email. I have no other options.

It doesn't sound like you using the mail merge function. First look up your contacts. Then on the menu select Write > Mail Merge. This will take you through wizard. See this Knowledgebase article for complete instructions:
