Sorry, I meant that if I have say 20 contacts in a look up, I can do a report that is just a list of Name, email, phone number.
I am trying to figure out how to do the same using a word template. But word wants to only do a new page for each contact.

Not an option. Word Processing templates are limited to one contact per page (or merge document is it is multipage).
