Looking for insight on mail merge docs

Hello
I am looking to do a mail merge document.  I seem to be having trouble.  I made the template document but for some reason not all the fields are being filled in and I can't get it to print correctly.  Some pages are missing info (fields) or my company info at the top of the page.
 
I would appreciate any insight anyone can offer.

Are you trying ro email or print the merge?
Which word processor - Word or ACT!?
Does it work ok if you preview?
Do the default templates work?

It sounds like your template has two different problems.
The pages with empty fields: is there actually data in those fields for those records?
Your company information, what do you mean? is it a graphic?
