The sender information for letters comes from the my record. Do a lookup and choose my Record. The information on this ACT record needs to be you. Many times we see people accidentally add the information for a customer here. This is incorrect. Add your info.
If it still does it, did you customize the template? Can you try another template? Does it yield similar results.
Finally try the demo database. If the demo database works and yours does not, you may need to replace the template in your database with the one from the demo database.
