Welcome to the ACT! Community. You would first need to install ACT! 2012 on the new computer, then register it with your current serial number. If you need your serial number, go to Help > About ACT! on your current machine. To move the database, follow the instructions in the following Knowledgebase article to backup/restore the database:

This has been discussed thoroughly on the board. Do a search on "new computer".
However, the best way is to use the backup service built into ACT and then "restore as" on the new computer. You will be picking a new name.
