Formatting labels not working

I'm trying to print out labels for an Avery 5160 template.  I have the following format:

 

C: Mailing Name

C: Address1

C: Address2

C: City, C: State C: Zip

 

I set C:Address2 to Collapse if Blank and Wrap Text.  I set C: City to Wrap Text.  However, when I go to print them out, there is always a blank line when there is no Address2, and City is always a fixed width.  Even when I start from a blank form and start over, it does this.  It seems like my formatting options are being completely ignored.  Any thoughts as to why?  I'm running ACT 2008 Version 10.0.3.182, Hotfix 1.  Thanks in advance

Don't allow fields to overlap or they won't collapse and you need to use a custom filed and a script to concatenate the city state and zip.

The fields currently do not overlap... can you be more specific about needing a script?  Is there an example you can share?
