Edit contact directory report to change size and include notes

I need a printed output of my data in an A5 paper size format (think Frankiin Covey address book) for an older friend who is scared of computers. 

I can edit all the fields and add fields just fine. However I cannot

1. change the size of the page to 5.5 inches to 8.5 inches

2. add the notes to the contact directory report

What would be awsome is of someone had cooked one of these up already.

In the report editor you can change the page size and on my system A5 is a page size selection. However the report editor isn't a word processor so be prepared to reformat the field layout because many of the field will be off the page. The field layout won't automaticaly adjust to the different page size. To add Notes ( I suspect that you actually mean History) you would need to add the appropriate sub-report to the template.

I don't have such a report on hand but I do custom ACT! reports as part of my business.
