Run a scan/repair using actdiag and try to convert the database again. If the same issue, create an empty copy of the database and import into the empty database. Follow the last KB if you have a multi user database to retain record manager.

Are the attachments in that location?
If they are not in the right folder, move them all to a set folder then you can use OakCheck! to fix the links in the ACT! database.
Otherwise, they won't be brought into the new version.
