Activity Report is displaying the same date and time for each contact

I have been using ACT 2012 pro. I recently went into activity report template to customize. At this time it was acurately displaying eah contact and the date and time on th calendar they were scheduled for.  I first made the mistake of deleting all of the fields at the heading of the report which caused the print preview to come up error "Guid should contain 32 difgits..." I read where this is becuase the contactid field was not on the report. So I added a contactid field to the header. Now the report prints, but all of the scheduled contacts have the same date as the first one. I feel this is a simple fix, but I don't know what to do.

The simple fix is to get a fresh copy of the original report template using Report | New Template. Default copies of the original reports can be loaded from there.

The simple fix is to get a fresh copy of the original report template using Report | New Template. Default copies of the original reports can be loaded from there.
