I've been through the knowledgebase here and searched the forum, here's the issue:
Creating a calendar item (appointment) in ACT! and it syncs fine with Outlook (2003)
Creating a calendar appointment in Outlook and it does NOT sync with ACT! 2012.
Can anyone point me in the right direction?

I'm not sure if this is relevant to your situation, but I believe that you can only sync with the default Outlook calendar (Calendar?).

Only using the default Outlook Calendar.
Had anyone got any ideas?

You will want to test the sync with first a new ACT! database and then a new Outlook profile. You'll want to backup your database and current Outlook profile before testing.
These tests help determine where the issue is contained - within the ACT! database or the Outlook pst file. Here is an article that contains instructions for creating a new Outlook profile: KB Article 12104
You may want to try running 'scanpst' to repair your existing Outlook profile; here is a Microsoft article with instructions: MS Article - Scanpst
