As I have for nearly 29 years or so, I have placed the commands (Company, Last Name, First Name, et cetera) to get around the Contact database on the toolbar. In 18.2 these settings are lost each time I am rebooted (automatically) in WIN10. What is the fix? I cannot get to my contact layout designer (even though I don't think toolbar settings are attached to a custom layout.

What is the full version of your ACT! v18 pro?
I suspect you need to get your ACT!v18 pro CURRENT by visiting the act download center at https://www.act.com/resources/downloads
I also suspect you may need to MANUALLY Check for Windows Updates. Start / Programs / Windows Update. or use your control panel.
After you set up your Custom Commands, Run a ACT! Backup / Personal files / Select Menus and Tools Bars ONLY ( check in box)..
Name your backup something like PersonalFiilesMenusToolBarsONLY ?
if you loose them, simply restore that special Backup.
Your message was not that clear. If you were referring to your Custom Column settings while in list mode, getting your act v18 current, should help resolve that issue if your list mode column display settings are not sticking.
NOTE: I get in my ACT Welcome Screen(not contact module) before I customize toolbars to help avoid hanging,
