I have recently re-installed ACT! and Outlook and completed the set-up for reintegration. Everything seems to work in terms of ACT! properly attaching e-mails to the contacts. However, prior to the re-installation, there was an ACT! Attachment icon on the Outlook toolbar. It doesn't seem to be there now. I have gone back through the set-up and it didn't seem to resolve the problem.
I didn't notice it was missing originally, but when I tried to implement the tip on how to attach incoming e-mail, I realized it wasn't there.
Does anyone have an idea of how to resolve this?

This happened to me, and someone on this forum had the answer. You have to go into Outlook, then add ACT to the Outlook address book. This knowledgebase article solved my problem:
