The failure to record history when using Word as the word processor was typically caused by the hidden text being missing from the template you're using. That will also keep you from doing a mail merge to more than one contact with that template.
The blank envelope was a frequent problem with ACT! 5 and 6 and the cause was never found nor the problem resolved.
