Clearing an activity for client A, will not post the information into history for client A.
However, i found the cleared activity in the history of another contact.
This was not an error on the users part, i have tested it, it continues.
I have performed the following:
-Ran ACT repair,
-Removed and reisntalled ACT
-Copy the database to a new database.

1. Does it happen every time?
2. Does it only happen in that database or can you replicate it in the demo database?
