the the custom buttons of 1,2,3 are not saved after program close and open
the toolbar however, is saved and appear blank without buttons after the program restart

It sounds like there may be an issue with the customisation preferences being saved.
Try clicking Tools > Customize > Reset Menus and Toolbars to reset the toolbar preferences. Recreate the custom buttons and test again.

Use the steps under the heading "Manually Rebuilding the Preferences".

Hi @Gary W , The solution at http://kb.act.com/app/answers/detail/a_id/14770 no longer seems to apply since ACT v22 for users who use Outlook. Unless I'm missing something, the Outlook Integration follows a different paradigm, and so using this method will result in duplication of contact records in Outlook.
Does anyone know of a solution that applies to v22 when using Outlook Integration???
