Word Documents not attaching to ACT! Contact

Using Word 2016 and Act V21 Update 9. When I try to attach a word document to a contact, nothing happens. The correct ACT! drop down is showing but does not launch contact list so cannot select the contact to attach the document to.

Check if Word has disabled / made Inactive anything related to the Act add-in from File > Options > Add-ins

The Act! Documents Tab and Act! Office addin were not checked. When I tried to check the Act! Office Add-in the message I get says: "Load Behavior: not loaded A Runtime error occurred during the loading of the COM add-in. The Act! Documents tab add-in won't load either, that message says "load behavior: load at Startup"
