Is it possible to have a smart task run based on a change in the status of a "STAGE" field in an opportunity? I want the smart task to run immediately and automatically, not have to wait for a certain time (i.e. 1am every weekday). For instance if a stage changes from "needs assessment" to "Quotation Request" I want an email sent to the sales manager immediately. (not have to wait for a certain time.

Smart Tasks won't initiate off of a change to a field, they are either scheduled or run manually.
After changing the field you can select Schedule > Run Smart Task > kick off the appropriate Smart Task and choose 'Selected Records' option. It will run immediately and only for the selected contact(s).
