Would having multiple shared mailboxes and the Act Outlook add-in cause Outlook to randomly stop responding and/or take a long time to startup?
Troubleshooting done:
Created new Outlook profile and found the same results as above.
Happening to multiple computers

Try this (EXACTLY as stated below):
Share the Demo DB on the Server where your main database resides.
Delete the default address book for the main from the user's Outlook integration settings.
Add the Demo DB address book (it then becomes the first one in the list AND the Default); User Chris Huffman , no pwd.
Add the main database address book as the second one (it then becomes the second one in the list AND NOT the default)
Uncheck the checkbox for the DEMO DB (NOT default anymore) and check the checkbox "default" for the main database.
This is how it should look like:
Test again.
This always worked for me in such cases, no idea why, so pls. don't ask.

The hosting server is running SQL Enterprise so the demo database is not present. Do you think I could just create a new blank database and try with that instead?

YES, the DEMO DB trick is just more convenient!
