Act! Premium Version 21.0.295.0, Update 5 with Office 365
When using a template to mail merge into an email frequently the address of a different contact is used. Sometime it affects the whole mail merge and other times it just some of the emails. The template has two field in it, <salutation> and <home address 1>, its only the address that seems affected.
