Input from this web page goes directly into a database that management has direct access to read, and which they also track and gather statistics on.

Not strictly true. You can add a field to your field list and then use an addon to copy the field to the new field and hence paste the two fields you require intot he layout. They are different fields but have the same information - so that might work for you.
Either try ABACUS addon from ACTpresentation.com or Exponenciel also have an addon. It really is up to you and how importnat it is to you to have the ability.

The problem with that is that if you update info in one field, you have to remember to do it in the second field.

It is possible to have a field more than once on a layout,
but it takes manual editing of the CLY file. It is a delicate
operation that could result in a corrupted CLY, best and
quickly done by an experienced professional.
