We are running ACT! Premium Version 21.0.295.0, Update 4 - shared database. My OS is Windows 7, using Outlook 2016 (Office 365)
We have email set up to record subject only for sent and auto-attach subject only for received emails. We are finding that some emails do not automatically update in ACT. It typically is the incoming (received) emails that are randomly not recording. There is no change in the incoming email address.
Any ideas will be appreciated.

Do you meet the requirements they have for office 365?
Office 365, as a web based service, is only supported on Act! v20.1 and later using the Act! Outlook Integration feature. If you are using v20.0 or earlier, the locally installed version of Outlook that comes with your Office 365 subscription is supported as long as you are using a local Outlook profile. Please note that if you are using an Act! version prior to v19, Click-to-Run installations of Outlook are not supported.
Local profile is probably the main one...
