User says contacts are missing and no one deleted them, I restore them from the backup and everything is fine but not sure why they are going missing. ACT! Premium 2010 12.1.181 HF4, user is an admin and she has modified the access list for the entire database. Are there any issues with doing that? Any idea where the records are going?

One possibility that bites people in the rear with ACT! - if you or the admin deleted a past user, maybe someone who is no longer with the company. When you delete the user, ACT! offers two choices: 1) Re-assign the former user's records to another user (sound logical) or 2) Delete all records belonging to the former user (wtf?)
If you don't read the options carefully, you can easily choose the wrong option and poof - all customers, leads, prospects, etc that belonged to the former user are instantly deleted.

If you modified the access list and now can't find people, that would point to problems with how the limited access changes were applied. Here are a couple of ideas. Make hte user a manager or admin and see if you can find records. Log in as other users and take a look.
Most likely the contacts are there.
