Can't edit ACT! Premium for web address book

Using Act! Premium (Web) Version: 21.0.295.0, Update 4.

I've used 'Update Act! Integration' from the Tools menu, but now I get the prompt to reconfigure the address book when I start Outlook. However, when I try to edit the Act address book in Outlook the dialog box doesn't display correctly (see below).

Did you try a different web browser? I mean ... If you currently work with Google Chrome ... then try Internet Explorer ..?  or vice versa...

Thanks for replying. However, the problem is present in Outlook, not the browser.
