Act Database Locked message in Outlook - Act2008 Premium

Hi - I keep trying to get my toolbar icons and addressbook functionality working in Outllok. When I load Outlook it says the act database is locked and do I want set it up. I click yes and outlook loads but does not offer the addressbook set up. I load act and go to Outlook setup and everything is there. If I deslect and reselect it makes no difference. Same problem whether I load Outlook first or Act first. Tried repairing Act and then Office goes into a long configuration routine. This does not fix the problem.

What version of Outlook?
What version of ACT! (if not 10.01, apply the 10.01 patch)?
 
Have you tried removing the address book from Outlook and re-loading it?
