Yup, there were 3 address books in the registry,
Did a detect/repair on Outlook to no avail, even did scan pst/ost to make sure there were no corrupt emails causing it.
couple of times a day no consistency on what he's doing when it happens. Sometimes, it's moving a email from one box to another, sometimes it's opening an email, sometimes it's creating a new email, sometimes it creating an opportunity in ACT.

You removed all 3 from the registry and now should be only one?
How many items in the Outlook Inbox? If a lot, try moving to another folder and see if that helps?
Odd that it can happen when creating an Opportunity - ie not doing anything in email.
Maybe try creating a new Outlook or even Windows Profile?
