ACT! 2009 for Financial Professionals Sudden High SQL Server CPU

What would cause Activities to suddenly go from taking less than 10 seconds to clear to taking nearly 5 minutes to clear with the SQL server process utilizing 100% CPU the entire time?

We can navigate contacts as fast as ever, just Calendar related tasks seem to take longer.

This seems to have started when 3 of the users started attaching email from Outlook. We have since turned that off.

We are using a 32 bit Windows Server 2003 with dual core 3.2 GHz Xeon processors and 4 GB RAM.

ACT 2009 for FInancial Professionals with all updates and hotfixes applied.

We removed cleared Activities older than 365 days and that offered some improvement.

I tried to restore from a backup to see if the same performance decrease was present before the Email Attaching began, but the restore as process errored out.

I am not sure where to go next here.
