I have a strange issue, it seems my plugin (which renames different menu items) only works if someone clicks the actsage.exe to open the program or a shortcut to that file. If they open ACT! by clicking on the .pad file of a database it does not run. Any thoughts?

Plugins are installed locally, so launching from the executable or from a pad file shouldn't matter. Does the pad point to a local or shared db?

Try adding your plugin to the plugin folder on the server and restarting Act.
Login there locally and ensure the menu item changes have been made.

I tried with a local database opening the .pad file and it did not run my plugin, I run the menu changes in the following event
Private Sub ActApplication_Open(ByVal sender As Object, ByVal e As System.EventArgs) Handles ActApplication.Open nb = ActApplication.Controls.Item("navbar")

I think these types of changes are usually recommended to be on in the after logon event.
Is it being added to dependentdlls.xml?

it isnt being added to dependentdll.xml I am not sure how to do that, but I will try moving it to the after logon event, by the way do you know the URN for New Secondary contact I tried many ideas including below but it didnt work, and on the sdk i cannot find it:

DependentDlls.xml lists add-ons that fail to load, I was just curious if it was failing to load your plug-in or if it was not working.
As for the urn for new secondary contact.... not sure. I completely expected it to be in the menu ID section of the SDK documentation... but alas, it's not.
