There are two easy ways to do this.1) export your contact list along with state, id status/ to excel using export to excel in the contact list, then try grouping in excel.
The second way would be crystal reports. This is a failry simple report there that most consultants can do pretty quickly.

The scripting to create the report in ACT! is very simple and I've done it several times. Frankly Crystal reports would be overkill.

The script would be specific to the database information. It would be one or more simple If - Then statements. As to placement, that would be specific to the report structure.
