If you wanted to specify formatting options for particular fields - how is it done?
For example, first letter captialized or ALL Caps.
I looked in Help and the Customer Support but did find much guidance.

You want to define fields and select the field type Initial Caps for the field you want to change.

I read it but it doesn't say how to specify formats as follows:
Smith (first letter is always capitalized with remainder in lower case)
SMITH (all letters are capitalized)

Under 'Tools' then 'Define Fields', create the field for which you wish to set these parameters. Change the default field type of 'Character' (in the drop down box below the name of the field) to one of the below:
Initial Caps (to create Smith)
Uppercase (to create SMITH)
Lowercase (to create smith)
